UNAIR NEWS – Dental extraction cannot be done without thorough preparation. A dentist needs a prediction formula to make sure when the dental extraction can be done. It is closely related with the maloccluded state of teeth.
Malocclusion is a serious problem. The causes can be genetic, malnutrition, extra teeth, caries, or mismatch between the sizes of teeth and the jaws. It is usually experienced by kids from 6-12 when they are at dental changes period.
Data of World’s Health Organization (WHO) showed that malocclusion is a priority problem in dentistry, after caries and dental tissue or periodontal. Prevalence of malocclusion in the teenagers are also quite high. Since 1983, prevalence of malocclusion had reached 90 percent, and in 2006 there was 89 percent.
In his open dissertation exam, Irwadi Djaharu’ddin, drg., M.S., Sp.Ort.(K) revealed that to deal with deteriorating malocclusion, incisor extraction is needed. It can be done to children at the age of 8 to 12.
On the other hand, there is still dispute on incisor extraction so dentists choose to postpone the treatment until all permanent teeth grow. But, the man born on July 8, 1950 believed that incisor extraction is a form of treatment for teeth which still exists.
“This treatment is useful to strengthen the muscles, avoid defect as early as possible, prevent caries due to overcrowded teeth and avoid worse malocclusion,” said Djaharu’ddin, during interview after the exam in FK Hall, on Tuesday, January 10.
The frequently asked question of general dentists is about when the extraction can be done. Through his dissertation, Djaharu’ddin has found a formula to predict eruption of permanent canines and premolars.
Eruption is the process of dental growth from being the seed and shows itself until it makes contact with the antagonizing teeth. Canines and premolars are the last grown teeth before molars and the third for having eruption. Canines are essential and have important roles for the facial esthetics as they form the angle of the mouth but they often do not get space.
Earlier method, eruption is predicted based on the age and the root growth but it needs more experience and special skill to make valid prediction.
The prediction formula Djaharu’ddin found used panoramic photo. In his opinion, panoramic photo is the standard and it is not affected by film position. By using panoramic method, the time to perform dental extraction can be determined.
Prediction of dental eruption can be made from four ways, observing the average of dental eruption, the growth of the root, correlating age, height, weight and the number of permanent teeth erupted.
To be able to find the method, Djaharu’ddin had collected a lot of data and studied it thoroughly. His research lasted for 28 years. In 1988, he found a dental eruption prediction formula using eruption index by measuring vertical distance. But his curiosity motivated him to keep finding better prediction method.
“I keep finding cases like this in my practice. It is varied and personal while most dentists do not really understand about it. Therefore I have always wanted to find a solution to simplify the dentists in predicting the right period to perform the treatment,” he said.
The man who has ever been the Vice Director of Dental and Oral Hospital, Faculty of Dentistry (FKG), UNAIR in 2008 hoped that his prediction formula can be used a reference in determining period of the incisor dental extraction.
The prediction formula is also useful to determine the antemortem data on a kid’s age when they experienced violence or had an accident. This method is helping the orthodontic forensics. “Because from seing the teeth, we’ll get the age information. And investigation through the teeth is more accurate, short and less expensive,” said Djaharu’ddin. (*)
